Personalized Weight-Loss Care Designed Around You
Losing weight can be challenging, especially when nutrition and exercise alone have not delivered lasting results. At NATT Weight Loss and Primary Care Services, we provide medically supervised weight-loss care designed around your health history, personal goals, lifestyle, and individual needs.
Our approach may combine medical evaluation, prescription weight-loss medication when clinically appropriate, nutrition coaching, lifestyle guidance, and ongoing follow-up care.
Whether you are beginning your weight-loss journey or need additional support after previous attempts, Alison Forsab, FNP-C, will work with you to develop a practical and personalized plan for healthier, long-term progress.
More Than a Number on the Scale
Weight management is not simply about eating less or exercising more. Your weight may be affected by several factors, including your medical history, medications, eating habits, activity level, sleep, stress, age, environment, and other health conditions.
Our medical weight-loss program looks at the whole person—not just the number on the scale.
Your personalized care plan may include:
- A comprehensive medical and weight-history review
- Evaluation of your current medications and health conditions
- Personalized weight-loss goals
- Nutrition and portion guidance
- Physical-activity recommendations
- Prescription medication when medically appropriate
- Regular progress monitoring
- Medication and treatment-plan adjustments
- Support for maintaining your progress over time
Treatment recommendations are based on your individual evaluation. Prescription medication is not appropriate for every patient and is never guaranteed.

Medical Weight-Loss Options
Semaglutide
Semaglutide is a prescription medication that belongs to a class of medicines known as GLP-1 receptor agonists. For eligible patients, it may be considered as part of a medically supervised weight-management plan.
Semaglutide may help support weight management by influencing appetite, fullness, and food intake. It is intended to be used together with appropriate nutrition, physical activity, and ongoing medical care.
Before recommending semaglutide, your provider will review your medical history, current medications, previous weight-loss efforts, health goals, and possible risk factors.
Your semaglutide plan may include:
- An initial medical consultation
- Review of your health history and medications
- Individualized treatment recommendations
- Nutrition and lifestyle coaching
- Gradual medication adjustments when appropriate
- Routine follow-up appointments
- Progress and side-effect monitoring
- Long-term maintenance planning
Semaglutide is not suitable for everyone. Your provider will determine whether it is an appropriate option for you.
Tirzepatide
Tirzepatide is a prescription medication that acts on both GIP and GLP-1 pathways. It may be considered for eligible patients as part of a comprehensive medical weight-management program.
Tirzepatide may help support appetite management, feelings of fullness, and reduced food intake. It should be combined with personalized nutrition guidance, appropriate physical activity, and regular medical monitoring.
Your treatment plan will be based on your health history, weight-related needs, current medications, treatment goals, and response to care.
Your tirzepatide plan may include:
- A complete medical assessment
- Review of your weight and treatment history
- Personalized nutrition recommendations
- Lifestyle and activity guidance
- Medication adjustments when medically appropriate
- Regular follow-up care
- Monitoring for effectiveness and possible side effects
- A strategy for long-term weight maintenance
Tirzepatide is not appropriate for every patient. A medical consultation is required to determine whether you qualify.
Other Weight-Loss Medication Options
Semaglutide and tirzepatide are not the only medical weight-loss options. Depending on your health history, current medications, weight-management needs, and treatment goals, Alison may discuss other prescription options with you.
Medication is prescribed only when medically appropriate. Your provider will explain the potential benefits, possible side effects, precautions, and alternatives before beginning treatment.
Never use another person’s prescription medication or purchase weight-loss medication from an unverified source.
2. Complete Your Medical Evaluation
During your appointment, Alison Forsab, FNP-C, will review your medical history and discuss the challenges you have experienced with losing or maintaining weight.
Your evaluation may include:
- Current weight and weight history
- Body mass index
- Current prescriptions and supplements
- Existing health conditions
- Previous weight-loss programs
- Nutrition and activity habits
- Sleep and stress patterns
- Personal health and wellness goals
Laboratory testing or additional medical information may be requested when clinically appropriate.
3. Receive a Personalized Treatment Plan
After reviewing your health information, Alison will develop a treatment plan based on your individual needs.
Your plan may include:
- Nutrition coaching
- Lifestyle recommendations
- Physical-activity guidance
- Behavioral goal setting
- Prescription weight-loss medication
- Follow-up appointments
- Primary-care support for related health concerns
Receiving a consultation does not guarantee that medication will be prescribed.
4. Begin Treatment and Follow-Up Care
Patients who qualify for prescription treatment will receive instructions for safely beginning their plan. Medication dosages may be adjusted gradually based on tolerance, progress, and clinical judgment.
Do not increase, decrease, stop, or restart a prescribed medication without speaking with your healthcare provider.
5. Monitor Your Progress
Follow-up care is an important part of medical weight management. During follow-up visits, Alison may review:
- Weight and body-measurement changes
- Appetite and eating patterns
- Medication tolerance
- Possible side effects
- Nutrition and activity progress
- Changes in medical conditions
- Challenges affecting your progress
- Whether your treatment plan needs adjustment
6. Develop a Long-Term Maintenance Strategy
Medical weight management is not intended to be a temporary crash diet. As you progress, your provider will help you develop realistic habits and a maintenance strategy designed to support your long-term health.
Nutrition Coaching and Lifestyle Support
Medication alone does not replace healthy habits. Our program incorporates practical nutrition and lifestyle guidance to help you make changes that fit your daily life.
Coaching may address:
- Balanced meal planning
- Portion awareness
- Protein and fiber intake
- Hydration
- Grocery-shopping decisions
- Managing cravings
- Eating away from home
- Physical activity
- Sleep habits
- Stress management
- Preventing weight regain
The goal is not perfection. The goal is to create healthier patterns that are realistic, sustainable, and appropriate for you.

Who May Be a Candidate?
Medical weight-loss treatment may be considered for adults who are living with obesity or who are overweight and have weight-related health concerns.
You may benefit from a medical weight-loss consultation when:
- You have struggled to lose weight through lifestyle changes alone
- You frequently regain weight after losing it
- Your weight is affecting your health or daily life
- You have a weight-related medical condition
- You want professional nutrition and lifestyle support
- You need ongoing accountability and monitoring
- You want to learn whether prescription treatment may be appropriate
Eligibility is determined through an individual medical assessment. Not everyone who requests medication will qualify.
